Deployment gates previously accepted one passing pipeline even when another failed or was cancelled. Both manual and automatic deployment now share latest-attempt evaluation and require every reported pipeline to pass; auto-deploy waits when no build exists for the exact revision.

Validation: 35 focused tests passed, including mixed pipeline verdicts, missing evidence, and successful retries. A broader web suite is running. This change evaluates stored evidence only; provider workflow discovery and authoritative completion coverage remain open release requirements.
