GitHub workflow hooks previously ignored requested and in-progress runs, allowing another passing build to make deployment evidence look green. Record queued/running attempts, keep reruns separate, and atomically reject stale transitions. Provider event time prevents a late old completion from taking latest-attempt credit.

Builds on PR #598. Validation: 15 webhook tests, a disposable database lifecycle test covering retries and out-of-order delivery, and web TypeScript passed. The database lane uses the existing PGlite PostgreSQL-wire harness. PR Lens diagram is local only.

This requires delivered workflow hooks and a matching changeset. Missing webhook reconciliation, expected-workflow discovery, and production qualification remain open.
